My Community & Home Care (MCHC)

Are you growing older and worrying about being able to stay at home? Are you living with a disability? Are you caring for a family member or friend at home?

The My Community and Home Care Program at Tallangatta Health Service is funded to provide a range of services designed to support you, or the person you care for, to stay active and independent and living at home for as long as possible. Helping people stay active, involved and doing as much for themselves as possible is proven to help people stay more independent with a greater sense of wellbeing. This is the approach we take in working with you.

District Nursing

Our District Nurse team is comprised of highly qualified nurses who provide general and specialised nursing and healthcare to clients to improve their health status and retain their independence to continue living at home.

  • What can be provided?

    The District Nursing Service provides assistance with:

    • Wound Care
    • Medication management
    • Assessment of health status including blood pressure and assistance with diabetes management
    • Palliative Care
    • Post acute care
    • Assessments for Domiciliary Nursing Benefits
    • Referrals for Aged Care Assessment and other health care professionals
    • Education for patients and carers about health care needs
    • Co-ordination of care with other health service providers
    • Continence management
    • Monitor general health
    • Blood pressure
    • Blood sugars
    • Blood collection for Pathology
    • Carers support coordination
    • Health assessments
    • Bereavement management
  • Who is eligable for care?

    Nursing services are offered to anyone requiring home based nursing care. Community members that would like to use the service are required to be assessed. Referrals can be made by self referral or by your doctor.

    People who receive benefits from the Department of Veterans Affairs, Workcover or Transport Accident Commission can also be referred to the service.

  • When will a nurse call?

    Following referral to the service a nurse will phone to make an appointment for an initial visit. This visit can take up to an hour. Carers are encouraged to be in attendance at this appointment as a plan of care is developed during this time.

    Subsequent visits will be made on nominated days, depending on needs. However, a specific appointment cannot always be made.

  • What programs are available?

    There are a number of programs you may be eligible for, including:

    • The Commonwealth Home Support Program (CHSP)
    • Veteran Home Care
    • Home & Community Care Program for Younger People (HACC PYP)
    • National Disability Insurance Services (NDIS)
    • Home Care Packages Levels 1-4

    The Commonwealth Home Support Program (CHSP) provides a range of support services to people over 65 (50 years or older and identify as an Aboriginal and/or Torres Strait Islander person) who are experiencing difficulties in managing daily tasks, but who wish to continue living at home.

Veterans Home Care

Services provided include:

If you are a Gold Card or White Card holder with low care needs, DVA’s Veterans' Home Care (VHC) program can assist you to continue living in your home by providing you with a small amount of practical help. The VHC Program also assists carers in recognition of the vital role they play in the veteran and defence community.

Home & Community Care Program for Younger People provides support for younger people with disabilities whose capacity for independent living is at risk.
